Not a pro-forma: the customer typed her own component list, and the frame number and her name are pencilled across the head of it, with prices worked up the right-hand margin in the shop's hand (90, 125 (100), 72 (50), 20, 25, 17, 12, 17, with “with block” and “SR” noted against items). The first attachment page holds her typed “Touring bicycle specification”, ticked through in the shop's hand with “Campag” written against the cable guides, margin figures 235 struck to 218, and on its lower half “my inside leg measurement is 29¾”” in the customer's hand beside the shop's “approx cost £600 see alternatives”, “Colour 078” and “8 weeks”; no shop pro-forma exists for the order. The second attachment is her covering letter of 3rd October, no year given, ordering a replacement for a bike she had stolen, asking only that the brake levers be easier to reach, colour to follow. With no year anywhere on the pages, year and order date are omitted.
