The fork ends were changed on the sheet, “Super Record” struck out for “Columbus with eye”. The surname is read from a fast hand as “Kulak” and could as easily be “Kwolak” or “Kurlak”; only the initial “O” is given for the first name. The finish is “To Follow”, so the colour had not been settled when the order was taken.
